Here is a strange one: the ball was sticking in the upper playfield arch, trapped by the cabinet itself.  The culprit was a right-side playfield support bolt which was too high in the cabinet.  Measurements revealed a 1/4 inch difference from the left bolt.  I routed the cabinet hole downward by 1/4 inch and dressed the fix with a flat washer.  Now the ball has clearance, and as a bonus, the playfield hooks (at the bottom edge of the playfield) engage in the cabinet bracket perfectly, whereas before I'd have to tug at it to make it fit.
